Appendix A

Periodic Test Records

The Periodic Test should be conducted every 30 days. The procedure for this

test may be found on page 5 of this manual.

The forms on pages 8 and 9 are provided for your convenience. They should

remain in the manual and be kept in a safe location as a permanent record

of periodic testing. Company procedures will dene how records are kept

and in what format.
