Appendix A - Periodic Test Records

A Periodic Test should be conducted every 30 days. The procedure for this

test may be found on

page 10 of this manual.

The forms on pages 13 and 14 are provided for your convenience. They should remain in the

manual and be kept in a safe location as a permanent record of periodic testing. Company

procedures will define how records are kept and in what format.
